Restoration with a three-tooth bridge

Mr G’s upper left incisor had fractured off leaving behind limited tooth structure, and the adjacent teeth were discoloured, heavily filled and overlapped. He initially saw Cathleen Perrin to discuss straightening with anterior alignment orthodontics, but she determined this unsuitable.

Mr G then saw Kit Spears, who performed a thorough assessment of his teeth and discussed his options: remove the tooth and leave a gap; use a removable denture; replace with a fixed bridge; or a dental implant. They also discussed teeth whitening to brighten his teeth, and correcting the alignment of his teeth with orthodontic braces.

 

Kit removed Mr G’s upper incisor, but Mr G did not want to leave a gap nor have a removable denture. Although a dental implant was an option, the time period (6–9 months) was not ideal for him, plus it did not address the alignment of the adjacent teeth and he did not want orthodontic braces.

 

It was decided that the extracted incisor would be replaced with a fixed three-tooth bridge. This would replace the missing tooth as well as improve the look and alignment of the two adjacent teeth. The usual disadvantage of a bridge is that it requires drilling into the adjacent teeth, which can increase their risk of needing root canal treatment. However, as both adjacent teeth had already had root canal treatment and were heavily filled, the risks were significantly reduced.

To help Mr G visualise the end result, a wax impression was made to show how the three-tooth bridge would look. This also enabled Kit to plan the minimal drilling required to fit the bridge, ensuring minimally invasive treatment, as well as visualising the final aesthetic result.

As both Mr G and Kit were happy with the tooth position and alignment, impressions and photographs of Mr G’s teeth were taken to construct the bridge, and temporary crowns fitted.

Mr G’s natural teeth were whitened with carbamide peroxide gel applied to bespoke mouth trays and worn for several hours each day until the desired shade was achieved.

The three-tooth bridge was made to replace his missing tooth and improve the shape, colour and alignment of the two adjacent teeth. It was made to match the lightened colour, translucency and lustre of his natural teeth, and trialled in place. As Mr G was happy with its appearance, fit and feel, the bridge was definitively fitted in place.

Not only did the three-tooth bridge replace a missing tooth relatively quickly, it also improved the alignment and appearance of the adjacent teeth, and Mr G is pleased with the result.

 

I am very satisfied with the solution provided by Kit and his team in resolving the issue with my front tooth that had broken off. The treatment was discussed fully beforehand, carried out painlessly over a period of time and the result was an improvement in my appearance.
